How to Apply for Social Security Disability Benefits

Applying for SSDI or SSI can feel overwhelming. This step-by-step guide explains the disability application process in Georgia, what forms you’ll need, and offers tips to help you strengthen your claim from the start. When & Where to Apply

Apply as soon as you become unable to work and expect your condition to last at least 12 months. You can file online at SSA.gov, by phone, or at a local Social Security office. No matter how you apply, accuracy and completeness are critical.

Information You’ll Need

Gather these details before applying:

  • Personal info: Social Security number, birth certificate, marriage details, and children’s information (if seeking auxiliary benefits)
  • Complete work history for the last 15 years, with job duties
  • Full medical provider list: names, addresses, dates, medications, and any test results
  • Clear descriptions of your impairments and how they limit your daily activities and work
  • Any other benefits you’ve applied for, such as workers’ comp

The Application Forms

Disability Benefit Application (SSA-16):
The basic application form—can be completed online or on paper.


Adult Disability Report (SSA-3368):
Lists all medical conditions, doctors, medications, jobs, and education. Be thorough and specific—details matter.


Work History Report (SSA-3369):
Explains physical and mental demands of your previous jobs. Highlight tasks that show why you can’t do that work anymore.


Function Report (SSA-3373):

Tips for a Strong Initial Application

  • Be consistent and detailed. Use specific examples—“can only stand for 10 minutes before pain” is better than “trouble standing.”
  • List every medical condition, even secondary issues—sometimes it’s the combination that matters.
  • Don’t leave out any doctors or hospitals—SSA will seek records from everyone listed.
  • Keep copies of everything you submit.

After You Apply – What’s Next?

Confirmation and Waiting:
You’ll receive confirmation that your application was filed. Most initial decisions take 3–6 months. SSA may contact you for more details or schedule a Consultative Exam (CE) with one of their doctors—always attend if scheduled.



Decision Outcome:
SSA will send a letter with either approval details or reasons for denial. If denied, don’t get discouraged—most people are. The next step is Reconsideration, and you have the right to appeal.
